SO, scenario in which Joker is a relatively healthy patient (young doctors only get those jobs) that totally has a crush on Harley, but she doesn’t give a crap about him because he’s 1. a wimp, 2. too low profile.
So, and as a consequence of the fact she’s a really shitty psychiatrist, he gets obsessed with her rejection and starts doing some memorable crimes so she can actually start taking him a little more sssseriously.
